Ready Steady Go programme
'Ready Steady Go' is a programme designed to help young people with the transition to adult age services. The programme is aimed at children and young people aged 11 years and over who have a long-term health condition and are likely to require ongoing support from adult services throughout their lifetime.
Advocacy services
The SEND Advocacy Service is an integrated advocacy service for children and young people in Southampton with Special Educational Needs and Disabilities transitioning (moving) from children’s to adult’s services, who are still in education. This service is available for children and young people aged five to 25 years old.
No Limits
No Limits offer free information, advice and help to children & young people under the age of 26 in Hampshire, the Isle of Wight, Portsmouth and Southampton. Visit the No Limits Advice Centre.

Hampshire Youth Access (HYA)
Hampshire Youth Access is a partnership of 11 leading agencies providing counselling, mental health and emotional wellbeing advice and support to children and young people aged 5 - 17 (or 24 for care leavers and SEND) across Hampshire.
