To consider the report of the Cabinet Member for Health and Adult Social Care seeking approval to consult on two interlinked proposals for an Integrated Service for Crisis Response, Rehabilitation, Reablement and Hospital Discharge.

Phase One: A proposed service model which will bring together those functions associated with hospital discharge, crisis response, rehabilitation and reablement delivered by the City Council and Solent NHS Trust, into a single integrated team approach, with a single integrated management structure, that better supports people in their communities and maximises their potential for independence. This preferred option has the potential to impact on numbers of staff posts.

Phase Two: Building on the re-design of Phase One, to offer a more flexible range of suitable alternative provision (bed-based accommodation or home based) that better meets people's needs whilst promoting independence and making efficient use of Health and Adult Social Care resources.  This option will have implications for the future locations of in-house services.
